Assumption of Risk & Release of Liability. I understand that athletic activities involve inherent risks including serious injury, illness, death, or property damage. On behalf of myself, any minor children listed, and our heirs, assignees, guardians, and legal representatives, I voluntarily assume all such risks and release, forever discharge, and hold harmless Freedom Community Ministries, Inc. (a Georgia nonprofit corporation) and MentorSports Gymnasium, together with their directors, officers, employees, agents, volunteers, successors, and assigns, from any and all liability, claims, and demands of whatever kind, in law or in equity, arising from participation, including injuries arising from ordinary negligence, to the fullest extent permitted by Georgia law. I understand that they assume no responsibility for or obligation to provide financial or other assistance, including medical, health, or disability insurance, in the event of injury or illness.Communicable Illness. I agree that neither I nor my children will attend while experiencing symptoms of a communicable illness, and I assume any risk of illness exposure that comes with participating in a shared facility.Emergency Medical Authorization. In an emergency, I authorize MentorSports staff to obtain medical treatment for me or my listed children if I cannot be reached, and I am responsible for any resulting medical costs. On behalf of myself, my listed children, and our heirs, assignees, guardians, and legal representatives, I release and forever discharge Freedom Community Ministries, Inc., MentorSports Gymnasium, and their directors, officers, employees, and agents from any claim arising on account of any first aid, treatment, or service rendered in connection with participation.Insurance Acknowledgment. I understand that, except as otherwise agreed in writing, Freedom Community Ministries, Inc. and MentorSports Gymnasium do not carry or maintain health, medical, or disability insurance for participants, and that I am responsible for my own (and my children’s) medical insurance and expenses. Should an accident policy ever be provided, it is secondary insurance that pays after my own insurance, is not a right, and may be ended at any time. Each participant is expected and encouraged to obtain his or her own medical or health insurance coverage.Georgia Law & Severability. I agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Georgia and shall be governed by and interpreted in accordance with Georgia law. If any clause or provision is held invalid by a court of competent jurisdiction, the remaining provisions continue in full force and effect.Code of Conduct & Check-In Photo. I agree to follow the MentorSports Code of Conduct. I understand a photo is kept on file solely to verify identity at check-in (passes are non-transferable); it is stored securely and is not used for marketing without the separate consent below.Photo/media consent (optional). MentorSports may use photos or video that include my player in its community materials.
